How to file a ClaimStudies have shown that the quicker a claim is filed, the faster the
Injured Worker will return to work and the lower the claim costs will
be. That is why it is so important to notify Medical Administrators, Inc.
as soon as possible after a workplace injury occurs.
If you have a username and password for our secure Client Access website, click here to log into the site. After you have logged into the site, click on the Report a Claim button on the left side of the screen to access the online form. To obtain a username and password, please print and return this form to us. Call our Intake department or your Claims Analyst at (440)899-2400/(800)542-9479 to report a new claim. When reporting a claim you will need to provide the following information: * Employee's Name, Address, Social Security Number, Phone Number, and Date of Birth * Date of Injury * Injury Description * Body Part(s) Injured * Treating Provider's Name, Address, and Phone Number * Date Injury was Reported * Last Day Work and if/when the Employee Returned to Work Fax a completed Report of Injury form to our Intake department or your Claims Analyst at (440)899-2411/(800)542-9480. You may use your company's report of injury form or print and complete this form. |
